Intake & referrals

Intake appointments are scheduled Monday-Friday. Referrals are scheduled daily for a full screening and assessment, and our team will guide you through each step so you always know what comes next.

Before You Arrive

Preparing for Your Stay

  • Residents should bring comfortable clothing and personal items appropriate for a residential setting.
  • Smoke breaks are available, and animals are not permitted on the property.
  • Phone times are scheduled twice daily so residents can stay connected with loved ones.
  • A physical exam is recommended prior to arrival to support your health and comfort during your stay.
